Bournemouth, Christchurch, and Poole (BCP) Council is pleased to invite providers to a virtual Market Engagement Event taking place on Tuesday 4th August, 11:30am–12:30am via Microsoft Teams. The purpose of the session is to outline our commissioning intentions for the upcoming Community Wellbeing Partnership Service. BCP Council is seeking provider views on the development of a new Community Wellbeing Partnership Service that will strengthen collaboration between statutory services and the voluntary, community and social enterprise (VCSE) sector, improve access to community-based support, and help residents to remain healthy, independent and connected to their communities. The service will support the Council's wider prevention ambitions by providing a simple route for professionals to access information, advice and community solutions that help people maintain their wellbeing and independence, including when returning home following a hospital stay. The Council is exploring a model built around three key functions: Professional Access to Community Support Providing health, social care and housing professionals with access to community knowledge, local expertise and information about available support. This may include: • A single point of contact for community-based solutions. • Access to VCSE knowledge and expertise. • Facilitating information sharing across organisations. • Maintaining resources that help professionals identify appropriate support. Individual Wellbeing Support Grants & signposting Providing flexible, practical assistance that helps people remain safe and independent within their communities and supports timely discharge from hospital where appropriate.
