Cardiff Council is seeking to understand the market from providers who are committed to promoting services that meet the needs of, and promote and maintain the independence and inclusion of, older people and vulnerable adults who may have dementia, other forms of cognitive impairment, physical and sensory impairment and or mobility issues, whose needs will range from targeted individual support to complex needs.
Extracare and Supported Living recognises that all older people and vulnerable adults, regardless of the level or type of care need, are able to make choices about how to live their lives.
These services enable individuals to live as independently as possible. Care and support is personalised and outcome focused. The vision is for a locality approach based on the benefits of care services working closely with preventative services, community health and social work teams, community hubs and primary care clusters to achieve the best preventative and care outcomes for people.
Extracare and Supported Living takes into consideration specialist requirements for people who may be presenting with other care and support needs such as dementia, other forms of cognitive impairment and physical/sensory/mental impairment.
The extent of the services provided are dependent on the varying needs of the individuals and their outcomes, which are identified through their initial and ongoing care and support wellbeing assessments. The services provision is based on services delivered to meet different levels of outcomes.
Individuals who receive the services are:
• Older people generally aged 55 and over
• Older and Younger adults with a physical/sensory/mental impairment
• People with Dementia or other forms of cognitive impairment
