Once-for-Scotland Digital Weight Management Platform & Virtual Care Pathways

This project will consist of three elements.

1.An innovative virtual weight management and anti-obesity medication (AOM) care pathway based on a shared care model,

2.A package of targeted innovation projects aimed at wicked issues within the weight management service that limit access to the service, patient management capacity and care pathway treatment capacity

3.A digital weight management platform

Virtual care pathway

A digital innovation model built around a virtual care based on a shared care model between the LWMS, Primary Care, Secondary Care and commissioned community partners utilising an end-to-end digital care pathway. It augments CDM care and can increase AOM deployment at a much lower cost than standard weight management care pathway models. The efficiency is the basis for scalability and sustainability and is realised largely by avoiding the additional Clinical MDT recruitment or commissioning within standard care pathway models. This has potential for large-scale deployment.

Innovation projects

Each innovation project is designed to mitigate a structural limitation, specialist/clinical staffing shortage or rate limiting factor that is a wicked issue that has been intransigent. These issues can be addressed through dedicated resource and staffing with solutions that could be deployed with core funding after OPIP to increase patient access, management or treatment capacity. These solutions include digital assets, automated assessment & screening tools and self-management resources that can be hosted within the digital platform, but aren't part of the core platform.
