1. Project name: "Construction of a Care and Treatment Facility in Polkowice, including rehabilitation facilities, a Magnetic Resonance Imaging Laboratory, and a Geriatrics Clinic." 2. The purpose of the investment project is to construct and provide a facility adapted to provide rehabilitation and care and treatment services [including inpatient and 24-hour services] that restore physical and mental fitness, allowing independent functioning at home, for patients requiring long-term and chronic treatment but not requiring hospitalization. The facility is to provide conditions of dignified treatment for people with a low degree of independence by creating rooms for long-term care beds, hospice and respite beds, as well as geriatrics, rehabilitation, and MRI rooms. 3. The objective of the contract, as set before the Contractor by the Ordering Party, is the proper construction of the designed facilities and obtaining the right to use these facilities. 4. The subject of the order is the performance of works and factual and legal activities, specified in the agreement and investment documents, ensuring the proper construction of the building of the Care and Treatment Facility [hereinafter referred to as the Care and Treatment Facility] in Polkowice together with rehabilitation facilities, the Magnetic Resonance Laboratory and the Geriatric Outpatient Clinic at the Polkowice Health Services Center [hereinafter referred to as the Care and Treatment Facility], as well as the construction of a container transformer station, technical and road infrastructure in the town of Polkowice at K.B.
