The subject of the order is the supply of disposable medical devices. A detailed description of the subject of the order, including information on the product range, quantities, and requirements, is specified in the pricing form constituting Appendix 2 to the Technical Specifications. The shelf life of the offered subject of the order cannot be shorter than 12 months from the date of each delivery. The product characteristics declared by the Contractor must be consistent with actual ones throughout the duration of the contract. Orders must be fulfilled in accordance with the requirements for the offered products. The subject of the order will be delivered at the Contractor's expense, risk, and transport to the Hospital Pharmacy – a warehouse located on the ground floor, with pallet delivery possible. The individual packaging of the delivered products must include the trade name, catalog number, series, and expiration date, before which the product can be safely used. Invoices issued or the accompanying delivery note must include the trade name, series, expiration date, and catalog number of the product. The offered subject of the order must meet the following legal requirements: the offered medical devices must meet the requirements specified in the Medical Devices Act of 7 April 2022 (Journal of Laws of 2022, item 974, as amended).
