1. The subject of the order is the delivery to the Provincial Hospital in Bielsko-Biała at 101 Armii Krajowej Avenue of laboratory reagents and consumables, according to the quantity and type specification specified in Appendix No. 2 to the Specification of Terms of Reference, hereinafter referred to as "swz", meeting the quality parameters specified in Appendix No. 2 to the swz, together with the lease of analyzers and a tube sorter for Part 3, meeting the operational and utility parameters specified in Appendix No. 2 to the swz, under the conditions specified in the draft provisions of the agreement constituting Appendix No. 4a to the swz for Parts 1-2 and Appendix No. 4b to the swz for Part 3. 2.
