1. The subject of the contract is the provision of group life insurance services for the Ordering Party's employees and their family members, hereinafter referred to as the Insured under Options I-XIII. 2. The subject of the contract includes: 1) providing access to the insurance program via an IT application, the functionality of which meets all of the following conditions: a) the ability to download all service forms from the application, b) the ability to submit monthly electronic reporting to the Contractor by the Ordering Party's insurance service personnel, including the registration of new individuals joining the insurance policy and the deregistration of individuals withdrawing from the policy, c) the ability to submit claims for undisputed benefits electronically (e-mail, fax) without the need to certify documents for compliance with the original. In special cases, the Insurer may request original documents, 2) providing at least two lifetime individual continuation insurance packages, differing in scope of insurance coverage and benefit amounts, in accordance with the minimum scope referred to in Appendix No. 3 to the Draft contract provisions constituting Appendix No. 6 to the SWZ, 3) enabling the Insured covered under Option I-XII to purchase an additional assistance services package, provided at least in the territory of the Republic of Poland, the price of which will be assessed as part of the offer evaluation criterion, and the scope will include at least the services indicated in Appendix No. 3 to the Draft contract provisions constituting Appendix No. 6 to the SWZ. 3. A detailed description of the subject matter of the order and the terms and conditions of order execution are specified in the Draft contract provisions constituting Appendix No. 6 to the SWZ.
