1. The subject of the contract is the supply and installation of medical equipment and apparatus for the Department of Urology and Urological Oncology and the Central Operating Block of the Provincial Hospital in Bielsko-Biała, ul. Armii Krajowej 101, 43-316 Bielsko-Biała as part of the task: “Improving the quality and access to highly specialized services in the field of urological oncology through the purchase of medical equipment and apparatus and the modernization and reconstruction of the Department of Urology and Urological Oncology” implemented under the National Plan for Reconstruction and Increase of Resilience – component D “Efficiency, accessibility and quality of the health care system”, investment D1.1.1 “Development and modernization of the infrastructure of highly specialized care centers and other medical entities”, including: 1) supply and installation of a thulium laser with equipment for the Central Operating Block – Part 1; of the types specified in Appendix No. 2 to the Specification of Terms of Reference, hereinafter referred to as "swz", meeting the quality parameters specified in Appendix No. 2 to the specifiers and under the conditions specified in the draft contract provisions constituting Appendix No. 4 to the specifiers. 2. Delivery of the subject of the order, together with transport, unloading and installation, will take place at the Contractor's expense and risk. 3.
