Poland – Miscellaneous medical devices and products – DOSTAWA WYROBÓW MEDYCZNYCH JEDNORAZOWEGO UŻYTKU

1. The subject of the order is the supply of disinfectants, cochlear implants, and voice prostheses used in ENT procedures (CPV: 33.63.16.00-8, 33.18.52.00-2) for Copernicus Podmiot Leczniczy Sp. z o.o. in Gdańsk, a detailed description, range, and estimated quantities of which are included in the Assortment and Pricing Form - Appendix No. 1 to the ToR (Parts 1-22). 2. The subject of the order is specified in parts No. 1-17, 20, and 22 regarding sales, parts No. 18 and 19 regarding sales with loan, and part No. 21 regarding commission without loan. 3. By completing the items in the "Describe yes/no" column, the Contractor confirms compliance with the requirements specified by the Ordering Party – if applicable. 4. The Contractor is obligated to provide a list of medical equipment – if applicable – with the first delivery. 5. The maximum delivery time for a partial order is 5 business days (taking into account the provisions in Chapter XVIII of the Technical Specifications – the delivery date is one of the offer evaluation criteria). 6. The offered subject of the order must be brand new, complete, of a high standard in terms of both quality and functionality, as well as free from material, construction and legal defects. 7. The offered subject of the order must be approved for trading and use in accordance with applicable law. 8. The subject of the order must comply with the Act of 7 April 2022 on Medical Devices (Journal of Laws of 2022, item 974) - if applicable. 9. In all provisions of the Technical Specifications and its annexes, in which the Ordering Party refers to standards, approvals, technical specifications or reference systems, or in which trademarks, parameters or sources of origin (names of manufacturers or devices) are indicated, in accordance with Art. 99 sec. 5 and Art. 101 sec. 4 of the Public Procurement Law, the Ordering Party allows equivalent solutions. 10.
