1. The subject of the order is the supply of disposable nonwoven medical devices and other products for USK-2 in Szczecin (hereinafter referred to as "products"). 2. The estimated quantities of products and a description of the products are included in the unit price form (Annex 2 to the SWZ, hereinafter also referred to as FCJ) and the required parameters form (Annex 2A to the SWZ, hereinafter also referred to as FPW). 3. The Ordering Party requires the provision of medical devices – within the meaning of the Act on Medical Devices (Journal of Laws 2024, item 1620, as amended), hereinafter referred to as "UoWM", placed on the market and used in the territory of the Republic of Poland in accordance with applicable regulations. 4. The Ordering Party requires that the delivered products have a warranty of no less than 24 months from the date of their delivery to the Ordering Party's registered office. 5. The Ordering Party has the right to order a smaller quantity of products, but no less than 60% of the quantity specified in the unit price form. 6. The order consists of 13 tasks. 7. The Ordering Party does not allow the submission of variant offers. 8. The Ordering Party requires the execution of the subject of the order for a period of 36 months. 9.
