Poland – Supervision of building work – Pełnienie funkcji inżyniera kontraktu dla zadania pn.: „Budowa nowej siedziby Komendy Miejskiej Policji w Katowicach – realizacja”

A detailed description of the subject of the contract can be found in the Special Terms of Reference (SWZ) and its annexes. The Contractor shall attach to its offer a declaration on meeting the conditions for participation in the procedure and on not being subject to exclusion, to the extent indicated in Chapter III, section 1 of the SWZ and to the extent indicated in Chapter III, section 2 of the SWZ (the declaration on meeting the conditions and not being subject to exclusion must be submitted in the ESPD form). Details within the scope indicated in Chapter III, section 2 of the SWZ: a) point 2.1. SWZ [regarding the grounds for exclusion under art. 108 sec. 1 items 1) – 6) of the Public Procurement Law] – the declaration should be submitted in the ESPD form; in accordance with the template constituting Appendix No. 4 to the SWZ. b) point 2.2. SWZ [regarding the grounds for exclusion under art. 7 sec. 1 of the Act of 13 April 2022 on specific solutions to counteract support for aggression against Ukraine and to protect national security] and Art. 108 sec. 2 – the declaration should be submitted in the ESPD form, selecting the appropriate answer in Part III, Section D; c) point 2.4. of the SWZ [regarding the grounds for exclusion under Article 5k of Council Regulation (EU) No 833/2014 of 31 July 2014] – the declaration should be submitted in accordance with the template constituting Annex 5 to the SWZ. These declarations constitute evidence confirming the absence of grounds for exclusion and compliance with the conditions for participation in the procedure, as of the date of submission of offers, temporarily replacing the required subjective means of evidence indicated in Chapter IV, sec. 2 item 2.1. of the SWZ.
