1.The subject of the order is the purchase and delivery of 1018 office chairs for the ZUS Branch in Lublin and subordinate field units, in accordance with the detailed description of the subject of the order and the terms of order performance contained in the contract template, constituting Appendices No. 1 and 5 to the SWZ. The subject of the order will be performed at the locations indicated below: 1) ZUS Branch in Lublin, ul. Tomasza Zana 36, 38C, 20-601 Lublin 2) ZUS Inspectorate in Puławy, ul. Lubelska 20B, 24-100 Puławy, 3) ZUS Inspectorate in Kraśnik, ul. Ks. J. Popiełuszki 3A, 23-210 Kraśnik 4) ZUS Inspectorate in Lubartów, ul. Lipowa 6, 21-100 Lubartów 5) ZUS Inspectorate in Radzyń Podlaski, al. Armii Krajowej 22/32, 21-300 Radzyń Podlaski 6) ZUS Inspectorate in Biała Podlaska, ul. Sadowa 23, 21-500 Biała Podlaska 7) ZUS Inspectorate in Łuków, ul. Kard. S. Wyszyńskiego 56, 21-400 Łuków 2. The scope of the order performance includes: 1) Delivery, unloading and carrying in office chairs at the Contractor's own expense and risk, in individual locations of the Ordering Party. 2) Setup in rooms, in places indicated by the Ordering Party. 3) Ensuring, at the Contractor's own expense and cost, the collection of packaging as well as the collection and disposal of old chairs. 3. The place of delivery and the number of delivered office chairs as well as the collection and disposal of old chairs are specified in the OPZ constituting Appendix No. 1 to SWZ 4. The Contractor is obliged to provide a guarantee and an equivalent warranty for the subject of the order. The Ordering Party specifies them for the period: 36 months for the seats and 48 months for the actuator (lifting mechanism) counting from the date of signing by the parties of the quantitative and qualitative acceptance protocol of the execution of the subject of the order without reservations, and in the event of defects being found, from the date of signing the final acceptance protocol containing...
