Capital medical equipment is one of the few categories where public buyers routinely and openly decline to buy the lowest price, because the purchase price is a minority of the lifetime cost. Evaluation models in this category are built around total cost of ownership: the device, the installation and any building work it forces, the service contract once the warranty expires, and above all the consumables, reagents or single-use items the device requires for the rest of its life. A platform sold at a discount that only accepts its manufacturer's consumables can be the most expensive option on the table.
Admissibility turns on regulatory conformity. Buyers require the device to carry the conformity marking or registration that applies where it will be used, in the specific configuration offered, and they check that the certificate covers the model and the intended clinical use rather than a family of products. Bidders also commit to a spare parts and software support period measured in years after delivery, to engineer response times, and to applications training for clinical staff at handover, all of which are contractual rather than promotional.
The stage that most often changes the ranking is clinical evaluation. Shortlisted devices are demonstrated to, and frequently trialled by, the staff who will use them, and scored on usability, workflow fit and image or output quality by a panel that includes clinicians. Interoperability is scored alongside it: whether the device writes to the hospital's record and imaging systems using recognised standards, and whether the data belongs to the buyer. A device that cannot be integrated is bought once and replaced early.
